HIPAA Privacy Practices Notice to Patients

Taylor Ann Drew, LLC is committed to protecting your privacy and ensuring the confidentiality of your health information. 3. Restrictions: You have the right to request restrictions on how your PHI is used or disclosed for treatment, payment, and healthcare operations.

4. Accounting of Disclosures: You may request an accounting of certain disclosures of your PHI.

5. Complaints: If you believe your privacy rights have been violated, you may file a complaint with us or with the Department of Health and Human Services.

Uses and Disclosures:

1. Treatment: We may use and disclose your PHI to provide, coordinate, or manage your healthcare and related services.

2. Payment: We may use and disclose your PHI for billing and payment purposes, including insurance claims and collection activities.

3. Healthcare Operations: We may use and disclose your PHI for our healthcare operations, such as quality assessment, training, and administrative activities.

4. Authorization: We will obtain your written authorization before using or disclosing your PHI for purposes not covered by this notice.
